INTERVIEW DOCUMENTS

You will recieve the IV15 form which lists all the documents you have to collect for the interview. The form asks you to place a check mark by each collected document to show that you have it in your possession. If the document doesn't concern you place N/A by the side (not applicable) The form asks you to have photocopies of each document expect for your passport and photographs. The documents you need to collect for the interview are as follows from the form IV15:-

  • Passport - Valid for at least six months, if you need to renew your passport, you can apply for one at the UK passport Agency.

  • Long form Birth Certificate - Showing your date and place of birth and your parent's names. Please note that the short form version of your certificate is not acceptable.

  • Adoption Certificate - If applicable, this was not applicable to me so I put N/A.

  • Deed Poll - Only necessary is you've changed your name. Wasn't applicable in my case, I put N/A.

  • Marriage Certificate - For each of your prior marriages. Again wasn't applicable to me, I put N/A.

  • Divorce Dcree or Death Certificate - Which proves that all prior marriages have ended.

  • Police Certificate - A separate certificate is usually required from the police of each country in which you have lived for twelve months or more after the age of 16. This certificate must state whether you have a police record in that country.

    I phoned my local police station and asked them to send me a form regarding the Police National Computer prosecutions/Convictions/Cautions History information. They told me to fill in the form and return it to my main police headquarters. You also have to include a fee of £10. You are advised to send the application well in advance as it may take up to 40 days. Mine took about 30 days.


  • Court Record - If you have been convicted of any crime, a copy of each conviction is required

  • Military Record - One certified copy of any military record should be obtained.

  • Two Color Photographs - These photographs must have a white background and must be glossy unretouched, unmounted and identical. The dimension of facial image should be about 1 inch from chin to the top of the hair; the subject should be shown 3/4 frontal view showing the right side of face with the right ear clearly visible. No earrings should be worn.

    Here's the photo of me that I send with the petition and that I'm using for the interview. I'm a little worried that the picture is too light, but our petition was accepted.

  • Evidence of Support - Form DS-2000, entitled "Evidence which may be Presented to Meet the Public Charge Provisions of the Law" The US sponsor must complete the I-134 form for those applicants who are providing addidavits of support as evidence that they will not become a public charge.

    The US Citizen has to provide evidence that they have income or assets that total 125% above the poverty line. You can find the exact dollar amount on this site :

    If you do not meet the requirements of being 125% above the poverty line. You will need a Co-Sponsor. The co-sponsor must meet the poverty guidelines for the persons in their household as well as the sponsor and the applicant. The co-sponsor has to fill in the I-134 just like the sponsor and has to provide evidence of support.

    Supporting Evidence - The sponsor must submit in duplicate (copy & original), evidence of income and resources, as appropriate.

    A Statement from an officer of the bank or other financial institution in which you have deposits giving the following details regarding your account:

    • Date Account Opened.
    • Total amount deposited for the past year. Bank computers don't store this information, so you need your statements for the past 12 months.
    • Present Balance.

    Satement of your employer on business stationery showing:

    • The date and nature of employment.
    • Salary Paid.
    • Whether position is temporary or permanent.

    If self employed:

    • Copy of last income tax return filed or
    • Report of commercial rating concern.

    You also have to have the I-134 notarised at the bank, this just involves someone stamping the form with a cool seal that leaves the form with an indent print. They also sign and date the form; this is just to witness that everything you've mentioned on the form is the truth.
