PACKET FOUR
On the 5th February 2004 Natalie received the Packet Four from the American Embassy in London. The unofficial looking letter, stated my Interview & Medical date and time. I had to be present at Dr.Phelan surgery at 8am for my medical. And I must present my letter at the surgery. A copy of my packet four letter is here.
The packet four also included instructions for the interview and also a reminder of what documents you need to collect for the interview. You also receive a nice looking map of the Embassy and the location of Dr.Phelan surgery, you can see my copy here.
You also get a payment slip. You must pay the visa application fee prior to your interview. You must pay the fee at your nearest Barclays Bank, or which ever bank they have stated to you. The bank will stamp your two receipts as proof of payment of the fee. Please be sure that you bring your RED "Embassy Copy" to the interview. Within this packet you also receive Medical instructions about the Medical interview, stating what happens at the medical and which vaccinations you have to be up to date with. You can find a full list of all vaccinations needed here. I phoned my local Doctors Surgery to obtain all my vaccinations records since I was born. Along with the Medical Instructions you also receive a Medical Questionnaire. The question's are as follows :-
- Have you ever been hospitalised (including psychiatric admission)?
- Have you ever had a disease of your heart or circulation?
- Have you ever had a disease of your lungs?
- Have you ever used tobacco?
- Have you ever had tuberculosis?
- Have you ever had a stroke, seizures or other neurological disorder?
- Do you have a major impairment in learning, intelligence, self-care memory or communication?
- Have you had any mental disorder or depression?
- Have you ever misused or abused drugs?
- Have you ever had an addiction to or abused alcohol?
- Have you ever caused deliberate injury to yourself or others?
- Have you ever caused deliberate damage to property or had trouble with the law because of a mental health condition, metal disorder or while under the influence of alcohol or drugs?
- Have you ever had any sexually transmitted disease?
- Have you ever had yaws, leprosy or malaria?
- Have you ever had diabetes mellitus or other endocrine disorder?
